Aura Integra Behavioral Health provides scheduled in-person psychiatric and addiction care in Worcester and secure telehealth across Massachusetts. Whether the practice is the right fit is decided together after an evaluation.

Before your telehealth appointment

Follow the secure instructions provided with your appointment. A scheduling confirmation or completed form does not guarantee clinical acceptance, a diagnosis, medication, or a prescription.

  • Confirm the exact date, start time, and Eastern Time designation.
  • Plan to be physically located in Massachusetts for the visit.
  • Choose a private setting and test your camera, microphone, and internet.
  • Have a photo ID available if requested.
  • Prepare an accurate medication list and preferred pharmacy information.
  • Gather only the records specifically requested for your evaluation.
  • Complete assigned forms only through the designated secure system.
  • Use the confirmed scheduling route if you need to request a change.

Two ways forms get completed

You can start on your own, or you can wait to be invited. Not every person receives every form. The forms assigned depend on age, service, payment arrangement, consent needs, and the information required for the evaluation.

Start it yourself in the patient portal

Creating an account asks for your first name, your last name, an email address, and a password you choose. Inside, a short pre screen and a demographics page are waiting, and your date of birth is asked there.

Completing those lets a chart be opened before your first conversation, which usually means fewer questions later. Creating an account is not an appointment, and it is not a commitment to be seen here.

  • Consent for evaluation, treatment, and telehealth
  • Communication preferences and patient-system acknowledgment
  • Medical, psychiatric, substance use, medication, and allergy history
  • Day-of-visit location and emergency contact information
  • Good Faith Estimate acknowledgment when applicable
  • Release of information when coordination or records are needed
  • Age-specific or service-specific forms when clinically relevant

Use the secure scheduling route to review current availability. For self-pay, language-access, form-access, or other nonclinical administrative questions, call or text 508-978-6455. Do not include medical information in an ordinary text or email.

A requested appointment change is not final until a new date and time are confirmed. Scheduled in-person and telehealth availability is confirmed directly with the practice.
